Powers of Attorney

POWER OF ATTORNEY

A document which authorizes someone else to act as your agent or attorney-in-fact.  The document is usually recorded in the Clerk of Court’s office.

 

DURABLE POWER OF ATTORNEY

A form of power of attorney which does not end in the event of incapacity.

 

HEALTH CARE POWER OF ATTORNEY

A document in which you authorize an agent to make health care decisions for you if you are unable to make those decisions.

 

LIVING WILL (DECLARATION OF A DESIRE FOR A NATURAL DEATH)

A document which applies to only two medical situations at the end of life (terminal with death expected within a reasonably short time if life sustaining measures are not taken or persistent vegetative state). 

 

**It is recommended that an attorney be consulted to assist you in the preparation and use of any of these documents.**
